Virgil, On "Aircraft Flight Report and Maintenance Record" from "21-06-2" (1962) at Langley Air Force base, signed twice in pencil, second American to fly in space and first person to fly in space twice. Grissom was killed along with fellow astronauts Ed White and Roger Chaffee during a pre-launch test for the Apollo 1 mission at Kennedy Space Center .
